Home and Community-Based Services

Home and community-based services provide opportunities for individuals to receive services in their own home or community. These programs serve a variety of targeted populations, such as people with mental illnesses, intellectual or developmental disabilities, and/or physical disabilities.

The Ohio Department of Medicaid offers the following home and community-based services:

Waiver Services
Medicaid home and community-based waivers allow individuals with disabilities and chronic conditions to receive care in their homes rather than in long-term care facilities, hospitals, or intermediate care facilities. More Information

elping Ohioans Move, Expanding Choice (HOME Choice)is a transition program that assists persons, age 18 and older, with any type of disability move from a long-term care facility (like a nursing facility or hospital) into a home and community-based setting. More Information
