HOME Choice: Information for Ohioans

Ohioans who need long-term services and supports get the services and supports they need in a timely manner, in settings they want from whom they want, and if needs change, services and supports change accordingly.

Home Choice services may include (but are not limited to) transition coordination, case management, independent living skills training, community support coach, nursing, social work or counseling services, and out-of-home and camp respite services.

To be eligible for HOME Choice, you must:

  • have resided in a Medicaid-funded facility for at least 90 days at the time of discharge;
  • have care needs evaluated by HOME Choice staff; 
  • qualify for Medicaid; and
  • move into qualified housing.

After you apply:  You will work with a Transition Team to coordinate the services and supports you will need, such as assistance in finding housing, coordinating benefits and getting referrals for additional services.  Learn more about the Transition Team.

If you are eligible, you may also enroll on a home and community based waiver program (programs that provide an alternative to institutional care) or receive other Medicaid services.
